Highland Medical, Primary Care Practices Receive Patient-Centered Medical Home Re-Certification

Nyack, N.Y. October 23, 2026 - Highland Medical, Primary Care Practices have once again been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program (NCQA PCMH). This recognition program is the most widely adopted PCMH evaluation program in the country. More than 10,000 practices, with over 50,000 clinicians, are recognized by NCQA. This model emphasizes patient-centered, team-based care, communication, and coordination, which has been shown to lead to better care and management of chronic conditions.

Over the last two years, NCQA and Highland Medical have partnered to work through the recertification process, which requires a primary care practice to learn and complete the six PCMH concepts. Throughout the process, NCQA conducts online check-ins to assess the practice’s progress and discuss the next steps in the evaluation. Practices that earn recognition through NCQA have proven their commitment to providing quality improvement within their practice and a patient-centered approach to care that results in patients who are happier and healthier.

“At Highland Medical, we are committed to providing an experience that is tailored to each patient and their needs,” says Yadira Castro, associate vice president at Highland Medical.  “We are thrilled to be recertified as a patient centered medical home program.”
